      Ordinary Minor Forest Products Permit

 

     
 
 

  1. Letter application or duly accomplished application form by the applicant.
  1. Application fee
  1. Sketch map of the area applied for forest products gathering
  1. If applicant is a corporation, association or partnership a true copy of its Articles of Incorporation/ Partnership and by-laws duly certified by the Securities and Exchange Commission (SEC)
  1. If applicant is a cooperative, a true copy of the Articles of Incorporation and by-laws duly certified by the Cooperative Development Authority (CDA).
  1. If applicant uses a trade name; a certificate of registration from the Department of Trade & Industry (DTI)
  1. A certified copy of latest Income Tax Return
  1. Proofs of Capitalization
  1. Indorsement of LGU concerned
